Required forms for participation

There are no student forms to participate.  But, we do have two critical forms that must be done by the coach.

First is the School Release form which in detailed in the next tab below this.  

Second is a form which combines three things.

1) You must fill out the form to indicate your squad's preference on streaming or not.

2) The head coach must verify that they have requested all their judges will do the read and consider the "Culturally Competent Judging" pdf by the NSDA.

3) You must indicate your method of payment that you intend. 

We would prefer both these forms are done, scanned and uploaded by Friday December 12. If you can't meet that deadline please let us know when we can expect it. They can be found and downloaded on the general tab of your team registration page.
