FORMS: School Consent & Single Entry Letter of Intent

During the registration process, you will be asked to download and complete TWO FORMS. You will need to download each form - 1 for the school and 1 for each member of your entry who is double entered. After you have completed all the forms, you will need to scan and email these forms to meghowell4n6@gmail.com.

You must turn in all of your paperwork no later than Monday, March 15th.
