School/Student Forms

All schools and students participating will be required to fill out an online tournament waiver form, linked below. One coach/adult chaperone from each school/institution registered must complete the "School Form". Every student registered must completed the "Student/Parent Form".

For Independent Entries, a parent may fill out the "School Form".
