Waiver and Permission Forms

Waiver Form

We have two waiver forms, one to be filled out by the school regarding supervision, the other to be filled out by the parent or guardian of each student.  These forms are posted as pdf files on the main tournament page.  Each school should collect the completed forms and submit them through our online portal:
