Tournament Attendee Form

Thank you for your patience while we streamlined the Tournament Attendee Form, formerly known as the Tournament Eligibility Form. Below is the Google form that everyone attending the tournament must fill out. Please read the directions carefully, and fill out all of the parts that are required for you as an individual in whatever capacity you will be there.

Coaches, please be thorough when filling out the proper name(s) and address for the awards to reach you. It helps us mail those to all of you faster when we aren't looking for missing information.
