Participant Eligibility Survey

All Persons in attendance at the Bargain Belt Invitational must complete this Eligibility Survey.  The survey will collect information that will help the tournament run more effectively.  The information collected will include a Liability Release form which is required by our Claremont Unified School District and must be completed by every person participating in the tournament.  This includes all student competitors, all judges and all coaches.
