Media Release Form/Eligibility Documentation

Media Release Form

Parents MUST sign the Media Release Form for their child. There is no exception to this policy. Entries without a signed Media Release Form will be dropped from the Tournament.

Coaches, please upload signed forms for each student to your Tabroom.com registration. Simply click "Edit Entry" then upload the Media Release Form for each individual student. If the entry is a partnered event (i.e., Duo, PF, or Parli), please ensure that the Media Release Form has been entered correctly for the corresponding student.

You may upload this to either the "Media Release Form" or "Eligibility Documentation".

You may leave "League" blank on the form.
