Required Liability and Chaperone Forms

Here is the NEW Code of Conduct form and Liability form that accounts for the new terrain of COVID-19. ALL parents, judges, coaches, and anyone who is attending in any capacity where one interacts with minors, must fill out these forms. Students must only fill out the Liability form.

If you would like to turn in electronic copies, the form must be compliant with authentication protocols programs such as DocuSign or AdobeSign are REQURIED, or just turn in the paper version. Return forms as soon as possible in a PDF or .docx, scanned, and emailed to DevenJackHowe@gmail.com.
