School Authorization Form

Submission Directions: To submit the school authorization form, click on the tournament registration. Then click on the "General" tab. You will see the "School Entry Forms" box on the right hand side of the screen. Click the Download button (little blue arrow pointing downward) to download the form and then once it is signed by the appropriate school official (principal or headmaster), please upload it on this page by clicking "choose file," find appropriate file and select, and then click "Go".  The document will then be uploaded to your entry. 

On School Letterhead, cut and paste the following EXACT text onto school letterhead and have a school official sign the document and submit to tabroom by Nov. 9th at 5pm (PT):

Our school is the degree-granting institution for every student from our school competing in the 2022 Damus Hollywood Invitational Debate Tournament hosted by Notre Dame and Harvard-Westlake High Schools. Each student represents the school. Our school is responsible for every student, coach, and judge attending the 2022 Damus Hollywood Invitational Debate Tournament under our school’s name and gives the students permission to compete and the judges permission to judge. There will be at least one school-approved chaperone present on each campus that your school has students competing on.  If our school is only competing online, then at least one school-approved chaperone will be accessible to the tournament staff throughout the tournament.  Our school understands that teams participating at the tournament may audio and/or visually record any competitor during debates.  It is the responsibility of coaches to ensure that their debaters and parents are aware of and understand that such a tournament recording is permitted. Our school shall be solely responsible for and complete all paperwork necessary to ensure compliance with applicable state laws regarding such recordings. We agree that if at any time the tournament discovers a student or school does not meet all the aforementioned criteria, the student(s) and/or school will be immediately disqualified from the tournament and forfeit any payments to the tournament.
