CCSD Required COVID-19 Activity Form

In order to be present at the tournament, all CCSD students must have a copy of this form on file with both their Coach AND their school's Activities' Administrator.

All CCSD coaches should have access to these forms on-site and should have access to contact information for all students' adult contacts accessible at all times.

Schools not in CCSD may have their own paperwork. All coaches at non-CCSD schools have an affirmative duty to obtain and manage their school's COVID-19 related paperwork for their students.
