Required Forms

1) Trusted Adult Form

Each school/program in attendance is required to submit a Trusted Adult form. This form intends to clarify the role of the person who will be the point of contact for your entry and remind the adult points of contact for each entry that this tournament will be COPPA compliant. Please make sure you complete this Google form by Sunday April 25th.

2) Media Release Consent Form

Every student competitor and their guardian must fill out a media release form. This form can be found here (as a printable PDF) or here (as a fillable PDF). Please feel free to have your competitors and their guardians sign this form digitally. If digital signatures are not possible, physically signed forms can be scanned into submit. We will only accept these forms digitally.

Forms for all students should be submitted to this link. Please remember that you must upload a form for EVERY student entered in the tournament.
