Contact Form

The TOC Contact Form:

1. Designates the person in charge of a school's entry and their contact information (Tabroom doesn't usually match what school's are planning). This person has to be the same as listed on the principal letter. This is the person the tournament will contact with any issues during the tournament related to a school's entries.

2. A shipping address for participant awards and any awards won. Our shipping process will be much faster this year.

3. Upload your school's mascot or logo. We will incorporate this into our video awards presentation.

4. Contact forms are due by Friday April 9th. It can be filled out at the following link:
